8 is a collection 800 of mobile/laptop/desktop devices
graphical user interfaces (GUIs) of physican version of MyTeleDoc application illustrating
a sample application of the MyTeleDoc application according to an embodiment of
the invention described herein. It should be noted that figures in collection 800
are for illustrative purpose only, and that additional arrangements and
functionalities can be incorporated without diverging from this concept of the
present invention.

In collection 800,
the process of invention’s coordinated care capabilities is showcased. When a physician 561 diagnosis a patient 112, and needs a second opinion of
fellow physician or input of a specialist, he/she can personally request to do
so via their MyTeleDoc application 655,
or instruct the provider 682 to book
an appointment with a physician or specialist. The physician/provider may

704 on their mobile/desktop device and may select, but not limited to, “Request appointment with a specialist” 805 from the menu options. The leads to

Specialist’ interface 810 in the

display area 704 may or may not
include the company name/logo 713. The different types
of specialists are further indicated in Figure
9. The physician/provider may proceed to select the type of specialist
needed 812 (using input mechanism 653 706) for consultation/coordinated and
this leads to the MyTeleDoc “specialist
selection” interface 820, and
available specialists are listed according to their available timings 823. The screen may also include, but
limited to, specialist’s photo, name, credentials and ratings. The
display area 651 704 may or may not include the company
name/logo 713. The physician/provider
proceeds to select a specialist, by either clicking on or selecting the
preferred specialist using the input mechanism 653 706.  The selection may include, but not limited to,
clicking on the image, clicking on select button, or swiping on the preferred
specialist using the input mechanism 653
706 in display area 704. Upon selection, the display area
may change to the MyTeleDoc “selected
specialist” interface 830 and

request for referral has been sent” 835

In Figure 8B,
the specialist may receive a notification on their desktop and/or mobile device
842, either (or through all) via their
MyTeleDoc app, or via a desktop MyTeleDoc application 844 or via an email. When specialist opens their MyTeleDoc
application 844 using input
mechanism 846, the display area 845 may
show a prompt stating the incoming referral request 852 in the MyTeleDoc ‘specialist request’ interface 850. The specialist may be instructed
to accept 854 or decline 856 the incoming service request. If
the specialist declines 856 request
or doesn’t accept request within reasonable time, physician/provider may be
requested to select a new specialist.

If the specialist chooses to accept 854 the request, the MyTeleDoc server 220 may connect the appointment to same
provider 682. The display area 651 704 may display the MyTeleDoc ‘confirmed
specialist appointment’ interface 860.

 In the display area 845, there might be
a prompt informing the specialist of the appointment, and prompt stating
patient, physician and patient have been notified 862. Notifications are send to the provider/physician requesting
the service and to the patient devices 502.

The display area 845 may or may not include the company name/logo.

Notifications 864 are sent to patient’s
device 502, physician’s device 652 and provider’s device 702.

9 is a
collection 900 are a list of all the potential physician/specialist available
on MyTeleDoc. It should be noted that listed specialties are for descriptive
purposes only, and that additional services(s)/specialties can be incorporated
without diverging from this embodiment of the present invention.
